Basketball Game Participation

• All clinic dancers are invited to participate in the half-time performance at the Basketball Game on Tuesday, Feb. 3rd, 6:45pm at Plano Senior High School.
• They may wear black leggings and will need to have neutral colored tennis shoes or closed-toed shoes for the basketball game performance.
• There is no cost for clinic dancers to attend the game. Parents may purchase tickets at the box office before the game if they wish to attend.

I do hereby give representatives of the Belles permission to seek medical help for my child in the event of an emergency.  I further understand that the Belles, PISD, and its representatives assume no liability should an emergency occur.  I also give permission for all photos and videos of my child obtained by the Belles be used in any and all media pertaining to the Belles and exclusively for the purpose of promoting the Belles. 

By selecting "Yes," I, the parent/guardian of the student named above, agree to release to the District the student's name, voice, verbal statements, portraits (video or still) and consent to their use by the District.

By typing my name above, I affirm that I have read all of the above information and agree to comply with the guidelines therein and allow my daughter to participate in the Drill Team/Dance Clinic.
